UPON SCHEDULING AN APPOINTMENT PLEASE NOTE THE NEW

CLINIC CHANGES AND COVID-19 PROTOCOLS

Please expect a PRE-SCREENING PHONE CALL / EMAIL the day before or the day of your appointment- screening must be complete in order to attend your appointment.

(See below for the screening questions, information, as well as signs and symptoms of COVID-19).

 

MASKS ARE MANDATORY upon entering the clinic. Please bring/wear a mask to your appointment. If you cannot provide your own mask, a mask will be provided upon entering the clinic.

 

HAND SANITIZER IS MANDATORY before entering and exiting the clinic and will be provided.

 

We ask that you please arrive ALONE and AS CLOSE TO YOUR APPOINTMENT TIME AS POSSIBLE.

 

If your practitioner is not at reception when you arrive, please enter the waiting area (to the left) where seating is provided in a socially distanced manner.

If you are considered high risk (have an existing respiratory condition, etc.) please take extra caution and consider all risks prior to coming for your appointment.

CASH WILL NOT BE ACCEPTED at this time.

All RECEIPTS WILL BE EMAILED at this time.

There will be PLEXIGLASS at the front reception desk to help prevent the spread of water droplets from coughs and/or sneezes.

We will be performing EXTENSIVE CLEANING AND DISINFECTING of the treatment rooms between every patient, as well as regular cleaning of all surfaces that come in contact with patients outside of the treatment room.

APPOINTMENT PRE-SCREENING QUESTIONNAIRE

  • Have you been in close contact with anyone with an acute respiratory illness or travelled outside of Canada in the past 14 days?

 

  • Do you have a confirmed case of COVID-19 or have you been in close contact with a confirmed case of COVID-19 without wearing proper PPE?

 

  • Do you have any of the following symptoms:

  • Fever?

  • New onset of cough?

  • Worsening chronic cough?

  • Shortness of breath?

  • Difficulty breathing?

  • Sore throat?

  • Difficulty swallowing?

  • Decrease or loss of sense of taste or smell?

  • Chills?

  • Headaches?

  • Unexplained fatigue/malaise/muscle aches (myalgias)?

  • Nausea/vomiting, diarrhea, abdominal pain?

  • Pink eye (conjunctivitis?

  • Runny nose/nasal congestion without other known cause?

 

  • If over 70 years old:

  • delirium, unexplained or increased number of falls, acute functional decline, or worsening of chronic conditions?

 

If all answers are NO = this is a negative screen and you can proceed to come for your scheduled appointment

 

If ANY answers are YES = this is a positive screen and you will be asked to reschedule your appointment and to contact Telehealth Ontario for further guidance.
